If this is your first time at Five Point, please download, print, and fill out the above paperwork OR make sure to arrive at least 15 minutes prior to your appointment time.

Cancellation Policy: We understand that things come up but we request that you please cancel appointments no less than 24 hours in advance. If you do not, you may be subject to a late cancellation / no show fee equal to 50% of the service fee, payable upon your next visit.

Refund Policy: No refunds on herbal formulas or products are ever administered. There are no refunds on packages once the first service is redeemed; however, the remaining monetary value on a package may be converted to a gift card for use toward another service or product at our clinic.

Insurance: Please note that Five Point does not directly bill insurance for acupuncture or bodywork. Payment is due in full at the conclusion of each service. If your insurance plan covers acupuncture or massage, we will gladly provide you with a superbill for you to submit to your insurance company in order to get reimbursed. Superbills are not available for community acupuncture, only private sessions.
